BUTLER TOWNSHIP — The following police activity report was received from the Butler Township Police Department. All subjects are innocent until found guilty in a court of law.

July 11

Subject reported the theft of money from his residence after having carpets cleaned by Stanley Steemer.

Report # 16-044220 Call date: 07/11/16 12:48 Primary Offense: IDENTITY FRAUD Walmart, 3456 YORK COMMONS BLVD DAYTON OH 45414

Complainant reported that a Sprint account was opened in the name of her business. Complainant does not know the suspect.

Unknown person made charges under victim’s credit card account without having physically possessed the card.

Suspect exited the business with three cases of beer and did not make any attempt to pay for merchandise.

Report #16-044487 Call date: 07/12/16 12:13 Primary Offense: DOMESTIC VIOLENCE_CAUSE BELIEF OF IMMINENT PHYSICAL HARM BY THREAT UDF 8733 N DIXIE DR DAYTON OH 45424

Responded to UDF on a domestic violence complaint between an employee and an ex-boyfriend. Suspect threatened to kill victim and then fled scene. Victim signed domestic violence charges on the suspect.

Subject reported the theft of his money that was kept in his bedroom at a group home. No suspects at this time.

Suspect attacked her father after he refused to give her money.

Suspect known to the victim attacked the victim and stole his weapon and money.

Responded to Motel where property was found. Property was recovered and owner contacted. Owner advised property was taken from vehicle from Max & Erma’s on 7/4/16, but not reported to police at that time.

Victims were hired to perform roofing work for the resident. During a conflict with the work and/or payment, the resident displayed on her belt a Dayton Police Department badge and handgun. She is no longer a police officer.

I began an investigation where it was reported by a father (ex husband) that his two children were shot in the legs with a BB gun by the children’s mother.

Accident reports

Miller Lane @ Maxton Road

Unit #1 was traveling north on Miller Lane in the north bound lane, and when at Miller Lane and Maxton Road attempted to make a left turn to travel west and in so doing, failed to yield the right of way to oncoming traffic and collided with Unit #2 which was traveling south on Miller Lane in the south bound lane. Unit #2 was removed from the scene prior to my arrival and parking in parking lot. Unit #1 when struck, spun outward and ended up backwards in its original lane of travel.

651 Martindale Road

Unit #1 was traveling west on Martindale Road in the right lane and when at 651 Martindale Road, Unit #1 lost control of the vehicle, ran off the right side of the roadway, ran into the ditch, and struck a fence and a large rock.

651 Martindale Road

Unit #1 was traveling northbound on Martindale Road and when at 651 Martindale Road, Unit #1 lost control of the vehicle, drove off the right side of the road, struck a culvert and the embankment.

2535 Little York Road

Unit #1 was traveling eastbound on Little York Road in the eastbound lane, and when at 2535 Little York Road, Unit #1 lost control of the vehicle, slid off the left side of the road, struck a concrete culvert, and rolled over completely into a ditch.

N. Dixie Drive @ Little York Road

Unit #1 was traveling northbound on North Dixie Drive in the left straight lane of travel, and when at approximately 250 feet south of the intersection with Little York Road, Unit #1 collided with the rear of Unit #2, which was traveling northbound on North Dixie Drive in the left straight lane of travel and was stopped in traffic.

7791 Peters Pike

Unit 1 was traveling north on Peters Pike in the northbound lane and when at 7791 swerved to avoid “something” and over corrected, losing control, going off the west side of the road, striking a mail box, several trees and going across a creek.

US 40 @ Union Air Park Blvd.

Unit #1 was traveling east on US 40 in the left turn lane and when at Union Airpark Blvd, failed to stop within the assured clear distance ahead and collided with the rear of Unit #2 which was traveling east on US 40 in the left turn lane and was stopped in traffic.
